Petechiae are tiny, flat, red, purple or brown spots that appear when small blood vessels beneath the skin leak blood. These pinpoint spots usually do not fade when pressed. While petechiae can develop after minor strain or injury, they may also indicate infections, low platelet counts or other medical conditions requiring attention.
Petechiae (pronounced puh-TEE-kee-ee) are small spots of bleeding beneath the skin or mucous membranes, usually measuring less than 2 mm.
Unlike many ordinary skin rashes, a petechial rash does not blanch (turn pale) when pressure is applied.
Petechiae may appear on the arms, legs, face, chest or inside the mouth. Their colour can vary depending on skin tone and how long they have been present.
The most recognisable symptom is a cluster of tiny red dots on skin that look similar to pinpricks.
Common features include:
Important: Seek emergency medical care if petechiae appear with fever and severe illness, confusion, a stiff neck, breathing difficulty or a rapidly spreading rash.
Understanding petechiae causes helps determine whether the spots are harmless or require medical investigation.
1. Physical Strain or Pressure
Intense coughing, vomiting, heavy lifting or straining can cause petechiae, particularly around the face, eyes and neck.
2. Infections
Certain viral and bacterial infections can cause petechiae. Dengue, a relevant concern in India, may produce petechiae alongside fever and low platelet counts.
Some serious bacterial infections can also cause rapidly developing petechial rashes.
3. Low Platelet Count
Platelets help blood clot. Thrombocytopenia (low platelet count) may lead to petechiae, easy bruising or bleeding from the gums and nose.
4. Medicines
Certain medicines, including anticoagulants and some antibiotics, may contribute to petechiae through bleeding or platelet-related effects.
5. Blood and Immune Disorders
Conditions such as immune thrombocytopenia (ITP), vasculitis and some bone marrow disorders may cause unexplained petechiae.
| Feature | Petechiae | Common Inflammatory Rash |
| Appearance | Tiny, flat pinpoint spots | Red patches, bumps or spots |
| Blanching | Usually non-blanching | Often blanching |
| Itching | Usually absent | May be present |
| Main mechanism | Bleeding beneath skin | Inflammation or irritation |
Not all non-blanching spots are petechiae. A medical examination is necessary to identify the cause.
A doctor examines the spots, reviews medical history and checks for fever, bleeding, recent infections and medicine use.
Depending on symptoms, recommended diagnostic tests may include:
Not every patient requires all these tests.
Petechiae treatment depends on the underlying cause, rather than the appearance of the spots alone.
Avoid self-medicating or stopping prescribed medicines without medical advice.
Seek prompt medical assessment for unexplained or widespread petechiae, especially with fever, unusual bruising, nosebleeds or gum bleeding.
A rapidly spreading non-blanching rash with serious illness requires emergency evaluation.
Petechiae are small bleeding spots that should not be ignored when unexplained, persistent or accompanied by other symptoms. Early medical assessment can help identify their cause.
Pathkind Labs offers diagnostic testing options such as CBC testing to support doctor-guided evaluation. Consult with your healthcare professional to determine which tests are appropriate.
